Welcome to our Wisconsin VFW Downloadable Forms page. All currently available forms are presented here, grouped by category. You may simply scroll down until you find the category you are looking for, or click on one of the general categories listed below to jump to that particular section.

• Most of our forms are PDF forms, which are universally read with the free download of Adobe Reader©.  If you don’t have a current version, CLICK HERE to download a free update.   
• Most of our forms include a “Typewriter” function that will allow you to “type” your information into the form and save it, print it or email it as an attachment. Follow the instructions in the Adobe program.   
• When other formats, such as Excel, are used, a notation appears with the name of the form in the drop down menu.                    
• All downloadable forms available are grouped by category and accessed by selecting from that category’s convenient Dropdown Menu. This represents a change from our former format which, while informative and convenient, nonetheless was a bit cluttered and more time-consuming to update. The new format will allow us to stay further ahead by being able to add or update current forms more quickly, and remove outdated material to reduce clutter.
