2012 MidWinter Conference - Complete packet to reserve rooms and meals for both VFW and Ladies Auxiliary Members attending. The form may be completed using the “Typewriter” function of Adobe Reader™. Complete the form, then print for mailing.

2012 Big Ten Conference Package - This packet contains everything you need to Reserve Hotel Rooms, Register for the Conference, place a supporting Ad in our booklet and state your preferences for the Lambeau Field Tour. Reservation deadline is September 24th, 2012. (This form incorporates a “Typewriter” function so you can enter information and print a form ready for mailing.)

Emergency Disaster Fund Application (2012) – VFW Members are eligible for financial assistance in emergencies from natural disasters, fire, etc. Application must be made through your Post. Post Election Report – 2011- This “short set” contains the minimum forms, with instructions, needed to report your Post Election Results and submit your Mandatory Delegate Registration for National Convention. Use these forms to submit by mail or fax. Quartermaster Bond & Canteen Manager Bond (2012) - National Bylaws require that ALL Posts carry a Quartermaster Bond sufficient to cover the maximum amount of cash assets under the QM’s control. If your Post has a Canteen, the Canteen Manager must also be bonded, as must all members who are authorized to handle cash accounts. This two-page form contains both application forms. Lillian Campbell Scholarship App (2012) - This is a Medical Education Scholarship open to any applicant with current or past Military Service, or directly related to someone with current or past Military Service. Wisconsin Post Veterans Service Officers Report – Post Veterans Service Officers are required to file a monthly report of activity. This form may be used as is and mailed to our Veterans Service Office in Milwaukee. We suggest that you use it as a worksheet and submit your data electronically, instead.
